ORDERING INFORMATION
All orders will be filled and sent out as soon as possible after we receive them.
Most orders will be shipped through the U. S. Postal Service. Dazor Lamps are drop shipped by UPS directly from our distributor.
We have tried to make shopping with us as easy as possible. When using our Shopping Cart you will have several options to choose from upon check out.
1. Use Secure Online Order Form.
To use your credit card to order online.
2. Use Create Mail Order Form option.
This form can be printed out and then:
a) Fax the form to us with your credit card information.
b) Snail mail the form to us with either credit card
information, a personal check or money order enclosed.
Note: For any new customers mailing in a personal check, your order will be held until your check clears the bank. After you are an established customer with us, we will ship your order as soon as possible.
All orders will be given a reference number.

Returns:
No returns on charts, floss, opened kits or Dazor Lamps.
(If Dazor Lamp is damaged in shipping or is defective, contact us for replacement.)
For any other items you are not happy with please contact us within 3 days after receiving your order and include your reference number.
